A Quality Improvement Initiative to Improve Pediatric Discharge Medication Safety and Efficiency

This article reviews a quality improvement initiative designed to improve the safety and efficiency of pediatric discharge medication education. It describes the integration of the electronic health record (EHR) with the MedActionPlanPro (MAP) medication teaching and adherence platform to simplify complex medication regimens and better support caregiver understanding and follow-through. Readers will gain practical insight into strategies that reduce discharge medication errors, enhance patient and family comprehension, and improve provider workflow.
