Series Description

This interdisciplinary session brings together hematology and emergency medicine teams to build a shared understanding of the challenges in transitioning patients with sickle cell disease from pediatric to adult care. Through a case scenario, presentations, system updates, and collaborative discussion, participants will explore the risks and complexities of transition, share frontline experiences, and identify opportunities to improve care coordination and patient outcomes during this critical period.
