Title: Standardizing the Inpatient Management of High-Utilization Patients with Functional and Psychological Needs

Objectives: 

  1. Apply a standardized, multidisciplinary approach to the inpatient management of children with chronic or functional conditions requiring longitudinal care.
  2. Identify and mitigate biases that may be associated with high utilization patients or may contribute to overmedicalization, including patterns in decision-making around testing, treatment, and hospitalization.​
  3. Establish a shared mental model among providers for planned admissions, prior authorizations, and care coordination to support high-value, goal-concordant care.
