  1. Participants will be able to summarize the purpose, data sources, and methods used in the hospital’s CHNA, and identify the key community health priorities that emerged from the assessment. 
  2. Participants will learn how the CHIP aligns with CHNA findings, outline its main focus areas and strategies, and understand how the plan guides the hospital’s community health initiatives.
  3. Participants will be able to identify specific ways they can collaborate on CHIP activities.
